Companion voucher not used/still available?

I have recently booked an open jaw redemption using a 2-4-1 voucher. The return was outside of the 355 date range so the booking was made with two seperate calls. My 2-4-1 voucher was used as part of the initial booking, but I have just discovered that (owing to the arse-about-face way BA refund/recharge for adding segments?) the voucher is now available in my account again, and is able to be used in a dummy booking. I have been charged the correct number of Avios for the companion booking.
I (obviously) have no desire to speak to BA about this and highlight their mistake - but just to manage my own expectations - what are the chances this is noticed? Does some kind of audit take place that may catch this at a later date? Or can I put this down to good fortune and look to use the voucher with minimal concerns?
