You are not so confused - you have it rigtht.

Correct - present Mexican passport to airline agent at check-in when departing the USA. Present Mexican passport to Mexican authorities when arriving in Mexico.
When departing Mexico, show the US passport to airline agent at check-in.
Yes, the airline will include passport info when it sends passenger manifest information to the US Government.
Yes, she needs to show her US passport at immigration on arrival to the US.
