In all transparency, I have not used ATMs the the CAI airport. However my wife an I travel to Africa at least once a year (and more recently about twice a year). We use ATMs for getting local currency. I always look for a real bank at the airport with a small office/lobby. Hopefully there is an ATM inside. If not I check out their ATM in the public area of the terminal and look for any evidence of tampering (you can search for spotting card skimmers on ATM machines on your preferred search engine) and use it if it seems OK.

So far (over about 20+ years) I've gotten a decent exchange rate and have never had a problem with getting my card compromised. And all fees (or differences in exchange rate) are dwarfed by the costs of exchanging money at any of the airport exchange desks. Away from the airport, I try to use only larger banks with ATMs for the same reason.
