At this point, I think you’re going to have to call them to try to use the credit. An agent should be able to scour through the history and find the original FFC and then access its residual amount. I’d try once or twice, and if that wasn’t effective, I’d try requesting a refund online to see what happens, and if it still doesn’t work, I’d file a DOT complaint. If nothing else, that would get a more experienced agent to look at your reservation, and hopefully they’ll look carefully enough to find the old credit.

I called United today. The reservation agent looked at the FFC and said he could only see $66. He pulled up trip I paid for with the FFC and verified the cost was $1467 but can't figure out where the unused $1k went. He said I can book a new trip with the $66. I pressed him about the $1k, noting that the original FFC was for $2.5k, so it has to be somewhere. He asked me for the original flight itinerary and I provided that.
He then went through it, saw the original amount, saw the new trip I booked, and agreed that there was $1k missing but that he wasn't able to help. I would need to be transferred to another department. However, I was out of time so I'll have to call back later and see what can be done.
