FYI, for multi person request, United asks:
If one person to act as authorized representative for all traveling party members, United requests that all party members need to sign a single form or separate forms that authorize said person to represent them; or, each person needs to send an email to United Customer Care with their request on what compensation they want with the case number cited.

I saw everyone who was traveling and had them sign a single form that authorized me to represent them in this case and attached to the email. They quickly processed and sent the ETC codes via email to the MP email addresses associated with each account. Each ETC is valid till end of 2023 and showed up in my online account and United app.
