Thought it would be worth an update on this one, I called back in and the agent I got through to (straight away this time, so must have actually been the gold desk) was amazed at the few things I mentioned and confirmed that I was indeed correct and who knows what the other one was doing. I asked her to check the voucher which she said also hadn't been applied, so maybe it would have gone through without being used... maybe not, moot point as it expires in 2 weeks anyway after having already been extended. In any case, the co-pay amount was confirmed to also be incorrect, that was per person not total, so all good - sent off, and new e-ticket receipt received about 12 hours later. Sorted.
