I think it's the aircraft certification and maintenance that is more expensive. You can train pilots for CAT III operations in pretty much the same time that you train them for CAT II operations.
For CAT III you need either autoland or a HUD.
completely agree that the non-recurring costs (the additional equipment on each jet, and the certification of the different configuration) are substantial … yes the boxes have their unique inspection/ functional test/ maintenance requirements, but I’m not sure those add too much to the baseline unless reliability (MTBF; Mean Time Between Failures) is a significant issue
