My one missing QR segment finally was credited today, eight weeks after the flight. It was initially "declined" by QR ("Member not found on flight"). It was resubmitted to QR with documentation and then subsequently escalated to the AIM liaison.

The boarding for this flight was like the fall of Saigon. Boarding passes were ripped, but apparently never scanned.
