imho it depends.

I had booking with cash+miles where price paid where similar to surcharges for award ticket but miles part was lower.

Actually, now when Skywards is not so good (small earnings, expensive awards) cash+miles (and family account) is my favorite option.

in old good days (2008-2010) best option was to upgrade, actually after LGW-DXB-SYD-DXB-LGW in economy I was able to upgrade one sector between Europe and DXB.
now this is not possible at all.
