OMAAT has been crying about missed upgrades for years now; if Ben wants to be up front, he should pay for it.
In recent times, there have been a few domestic markets where the premium cabins have been sold as Flagship Business and Flagship First. I will summarize below.
Flagship Business (no Flagship First)
772 and 788/789 equipment flying MIA LAX, DFW* HNL/OGG
*Note: In the last 2 years, from time to time, AA has deployed 777-300 to HNL but in that case, both the true F and J cabins were sold as Flagship Business
Flagship Business and Flagship First
777-300 MIA LAX (however, Flagship First is no longer available year-round since AA isn't always offering 777-300s on this market)
A321T JFK LAX/SFO/SNA and BOS LAX

Mostly true but not fully accurate.
DFW HNL/OGG and PHX HNL/OGG sell the Premium Economy cabin as such, so not free for EXPs and CKs in advance, though often free on the day of departure when available.
