It does confirm that two accounts were opened, but the one in your employee's name will be a mess if it is linked to your SSN. Business cards are issued to individuals, not to companies. It is probably linked to a separate Membership Rewards account in the employee's name. I would close that one, keep the one in your own name, and order an employee card on that account.
