Yes, but if using verifly you can just scan the qr code for your last jab and that is sufficient, no need to upload anything. Beyond that no one will ask to see it, but obviously prudent to have the certificate on your phone or the jab passes in your apple wallet or hard copy.
Is there an Attestation Form that I need to print before and complete by hand?
Again if using verifly you just tick a statement and no need to do anything with any physical form.
