I applied online on the American Express website. I used my regular mailing address, and then called to advise them that it needs to be sent out of the country. There's a specific department that handles international mailing. They just verify the address and confirm that FedEx can ship it and it arrives in less than 2 days. A replacement card on a different Amex account was sent to me in 2 days also internationally.
