I just completed 4 segments on JAL last week, passport has middle name, ticket did not - not a peep or comment from anyone. As a matter of fact, from United, Delta, Air Canada, Lufthansa, Swiss, and ANA, I have never included my middle name on my ticket and never had a problem, ever.
