I heard from GOTBound about a case that could shine some light on some of the issues with upgrades. Perhaps mainly the situation where upgrade seemed to have cleared but ticket was still on waitlist (a.k.a. "ticket not updated" issue). To my understanding, AY rep said that waitlist clearing didn't work because of married segments. Ie waitlist process had released seats on each of the segments, but in an un-married fashion. The waitlisted ticket is married though (as all connecting tickets are) and thus ticket could not use the release seats.

Solution was to un-marry segments in the original ticket, and operator must ask supervisor for this. Once un-married (the "update" process?) the ticket can immediately use the available seats.

This explains much. It might even be a part of the "T-37 rejection, but 're-waitlisting' clears the upgrade", perhaps there are similar issues of available seats that the automated waitlist process sweeps does not see.
