Solution was to un-marry segments in the original ticket, and operator must ask supervisor for this. Once un-married (the "update" process?) the ticket can immediately use the available seats.

This explains much. It might even be a part of the "T-37 rejection, but 're-waitlisting' clears the upgrade", perhaps there are similar issues of available seats that the automated waitlist process sweeps does not see.
Another data point for similar issue what I observed with the HEL-SIN flight. I was on waitlist to J upgrade with points which was denied T-37h and I flew in economy. However in my app the carry-on allowance changed from 1 carry-on + personal item (8Kg) to 2 carry-ones and personal (10kg). The non used points were not automatically refunded but needed to separately request to get them back.
