Getting the initial VoA is straightforward, fast, simple. The extension is more complicated, certainly can't be done when picking up the initial one. You can do it in Bali, indeed anywhere there's an immigration office. You can save money by cutting out the agent and doing it yourself: but agents know the ropes, have friends in the right place, and take the headaches and faff out of the process
