Basically premium flights are slightly nicer than regular F flights in food presentation and some types of spirits. Beyond that, premium is not really all that special on AA.
I totally disagree. AA's premium service may not be up to the standards of the Asian and many European carriers, but it's far better than the non-premium service. The OP's wife most likely will get a flat bed seat on an Atlantic configuration 777 (AA's best seat by far, comparable to the best seats of any airline, and, in my mind, reason enough to blow the 125k miles), enhanced meal service (it's far better than anything else on AA, but it's still airline food), and a built-in personal video unit with Bose headphones. To the OP: make sure you wife asks a flight attendant for instructions on using the seat: it has enough bells and whistles that it can difficult to figure out how to use all of them.
