If you have filled it out properly, you should have a receipt with a 6-character alphanumeric code and a QR code. Beneath that it will list the travelers and their vaccine status. If only one traveler is listed, I’d delete it and start again (or just begin it a second time and select only yourself). You do need to have this done before boarding. My guess is that somewhere along the way, when it came time to select travelers, you selected only her.
