We used J-G-A for the EFRS. Zero issues. Very fast service and prompt replies to our questions.

A few things to note:
1. The NY consulate didn't care that our EFRS has no middle names. Our Visa was processed with our middle names.
2. Upon arrival at Haneda there is a comical number of agents meeting guests and pointing them to the Covid check area. It's like 50 different employees saying "Welcome" on the way there.
3. We used the MySOS app. We uploaded pics of our CDC cards before our flight. Turned blue in about 10 minutes.
4. Covid checks took 2 minutes - there are probably 30 agents checking the MySOS record. Very simple process. They hand you a pamphlet on Covid rules.
5. Immigration took exactly 2 minutes - same as it always took in the past. They didn't ask for anything other than the evisa mobile page to compare the picture to the human. They scanned our passport like always, affixed the stamp, etc.
6. Customs was more thorough than normal - they asked a lot of questions but the process took about 2 minutes.
7. Non-Japanese seem few and far between around Tokyo. Everyone seems very excited to practice their English.

Zero issues on any of this process. Very simple. Going to repeat it again over Thanksgiving.
Did you need an "itinerary" document at any point, either for the visa or upon check-in or arrival? Or did the whole process involve only the (e-)documents you explicitly named here (EFRS, visa, MySOS, passport)?

Likewise, any questions about insurance?
