We booked our flight and hotels on our own and purchased a EFRS thru a travel agency in Japan. We just applied for a visa from the USA and will see how that goes. It seems pretty simple - but you never know. The visa application took about 15 minutes to figure out and upload the passport and picture and EFRS certificate. Hopefully Visa will get approved and then we can go to Japan pretty much independently for a trip in November.
