Rarest Type(s) of an Otherwise Standard Airframe

So while I was digging through some paperwork from a former MRO company I ran, I found some interesting notes about the 747-100BSR SUD. This is a 747-100 built for short haul routes for JAL's domestic network. It takes the SR variant (which is designed for higher tonnage and reinforced for a higher number of takeoffs and landings - and is also a JAL specific trim) and combines that with the stretched deck of the 747-300. JAL ordered exactly two of these birds.

Can anyone think of some other neat examples of one-off or a couple-off examples of otherwise normal airliners like this? Would be excited to learn about some unicorns I've not encountered yet.

A great photo example uploaded on Wikipedia.
