For what it's worth I also paid 20,000 yen to JGA for an ERFS for two people for a November trip. I received the ERFS certificate on Wednesday evening and immediately applied on the eVisa website.

In both cases I was NOT asked for an itinerary. Both JGA and the eVisa website only asked for my first night's hotel and address.

Initially, my eVisa status read "Temporarily Received." This morning it has changed to "Examined and Passed," and the eVisa fee changed to "Exempted." I have not received any email from eVisa; I know this by logging in and checking the status.

Honestly, and this comes from someone who likes to have all their i's dotted and t's crossed, I am not very concerned about entry. I think if I have the Visa and the ERFS from the receiving agency there will not be any issues at immigration or once in country.
