IMO, #2. My last claim with AMEX was a cancellation due to weather and I booked my own hotel overnight, then continued home the next day on the new flight they rebooked me on. I filed a claim and it was reimbursed with no issues.

the terms of the insurance don’t seem to cover award tickets, even if you paid taxes/fees with card (though I’d love to be proven wrong - I don’t recall any examples of award tickets being covered, but it is possible I missed one, too). While there is a provision to cover tickets which were purchased using vouchers, etc. with the balance on the card, there is no provision which talks about awards. While it’s possible they are intended to be covered, my suspicion always leads me to believe anything that is not specifically mentioned is more likely than not to be covered. That said, it’s always worth filing and taking a shot. The worst they can do is deny the claim
Chase does cover these - I can say this from experience. Their terms are much more clear about only a portion of the cost of tickets needing to be charged to the card than Amex.
