I'm glad my regular medical insurance covers international travel, because I've ended up in emergency rooms (non-covid related), twice while traveling. They want cash or credit card on the spot before you leave. And they want a guarantee benefits before they even look at you. You can easily be looking at least $5000 just for the emergency room, unless it's something very minor.
