Yes it would be interesting to know if this was a booking that had been amended a few times, and hence some keywords dropped out along the way. It's not something that necessarily happens regularly, it's not as though this forum is flooded with examples of this, but it wouldn't surprise me if it was a series of "bad luck" stories all rolling into each other. Had the OP gone HBO or had it through checked to ORD probably everything would have been fine even though the ticket may have been just as flawed as it was here.
Yup, that's what I'm thinking. cancelling and re-issuing a bag tag is a basic thing that the lowest of A3s, as I was once, can do and do. But if the booking is altered in some way, then it's damn hard to change it. In the case I had it looked as if the passenger had already exhausted his bag allowance, for instance.
