I've been wondering the same question. In fact, I wrote today to one of the "Cancel for Any Reason" policies and specifically asked this question. Those policies are incredibly expensive, and they often have "medical evacuation" built in. So my question was whether or not they would honor the cash price of a return ticket in the same cabin as what was booked. No answer yet.
