Do most airlines pay for a hotel if the flight is delayed or cancelled?

The weather has been terrible this summer with lots of late afternoon thunderstorms that have caused long flight delays and cancellations. It was always my understanding that if the flight is cancelled, even for weather, the airline would offer a hotel voucher for a place to wait out the weather and get some sleep before the next day's flight.

When have you been able to get a hotel voucher if your flight was delayed or canceled overnight? Did you get a hotel voucher for:

Weather delays or cancellations

Pilot or Flight Attendant Shortages

The Pilot ran out of flying time

The plane is broken and getting it fixed was a huge undertaking

Air Traffic Control issues.
