As for accents, just go to Scotland. I've always had trouble understanding English there!
Well I was once on a train in Scotland and a family who were dressed like and sounded like German tourists boarded at Prestwick Airport, only when the conductor went and started joking with them did I realise they were very local.
The engine was extremely noisy and it really sounded like they were speaking German from 8 rows away. Being curious I moved closer and from 4 rows away I could now tell they were speaking English with a west Scotland accent, but I still couldn't understand a word until the engine cut and it was quiet enough to concentrate on what they were saying.
