Missing a subtle defect like the “Dr” thing is much more likely to be an outcome of this, rather than any unconscious bias in the testing team.
It's a fair comment that this is likely to be pure incompetence rather than intentional bias. But you are making a distinction that doesn't matter. If something is wrong it's because someone made a mistake. They almost always will have made that mistake innocently, but what is the root cause of the error?
