I suspect some of it is logisitics - you need a reconfiguration to allow some arrival streams to be processed into departures without mixing with "dirty" passengers from non trusted locations. Airports like Frankfurt and Schiphol have this baked into their design whereas LHR etc doesn't
Well LHR does actually do it with domestic connections, just send connecting pax up the escalators to the departure level. If they do it for domestics no real reason it can't be done for other flights. Could even be selective so it's available for "trusted" arrivals, but not others.
Of course doing so would take pressure off flight connections, but we don't really want any of that do we
