I don't see refund-level issues here. Definitely post on this stuff in the reviews and hammer them in the survey if you get one (hint: for those surveys #s hurt the metrics, words don't).

To be honest, everything you've listed is a minor thing to me. Something I'd mention to the front desk, but that's about it. Then again I'm a road dog, away from home far more often than being home. To me the biggest offence here was the clogged sink. That should have been caught by cleaning staff whilst doing their cleaning. The mold shouldn't be there but is minor. As for the shampoo/body wash issue, the front desk should have given you some on-the-spot. If not, help yourself to the nearest maid cart/supply closet.
