The one I mentioned above includes up to $250 award cancel/redeposit fees. I've seen others that have something similar. But they won't help of course with needing more miles for a new award ticket. So if a new flight booking is needed due to a covered reason, you'd want to just buy a cash ticket for later reimbursement by the insurer. Keep meticulous documentation of everything.
A second vote for Travel Insured. They have a written "Covid Clarification" addendum that specifically spells out how their coverage works with Covid related disruptions. It doesn't cover routine covid tests as needed to meet travel requirements, but once you test positive during your covered trip dates, the things they cover are fairly standard, with limits. I also use insuremytrip.com, as they have a selection filter for Covid coverage.
