You can do FQTV by pax/segment, but other than (pre-Amadeus) one agent who thought that was the only way to do it (which resulted in one segment not having a FQTV), every agent and every system I've seen has just done it by pax.
I've done by segment at booking regularly for the last few months and everything has been working fine. I've never seen it on an airline or OTA website though.
