If you want a PAPER attestation - normally if you use Verifly and/or OLCI you declare then the same info in this form. Some outstations it comes in handy vs arguing about Verifly
Ah, that makes sense! Last time I flew was in August and I did all the necessary stuff through United's app. Flying AA this time and was concerned that now I have to print out a paper form - if AA uses an app hopefully i can jam everything in there.
