So I am asking YOU, why was I denied access to the lounge on my second visit.
Asked and answered -- Access on arrival to a Polaris Lounge requires arriving on UA operated (not UA marketed, but operating) flight. If not, no access.

If the previous flight was a departing partner flight, access is provided per *A rules. Or access could have been in error -- regardless, there will be no access on the arriving partner flight per UA and *A rules.
