If you often go to the USA it can make sense due to the insane hospital costs.
I remember the ceiling for the VISA Premier I had was around 160k euros.
It could be interesting to see the fine prints of travel insurance when you buy a flight ticket.

It is way cheaper (9,90 euros/month with an Allianz contract which is one the best) and you are not stucked with AF-KLM. They also don't have any FX fee. The Metal one at 16,90 euros/month comes with a few extra insurances.
