Yes you can bring a backpack still (a personal item for under the seat). Usually what I do is put a couple of extra heavy things in my backpack so my carry on is definitely under the 18lb limit and then move it to my carry on after security.
And for how picky they are, it depends if you get lucky or not. I've had people complain, and I've had people say whatever.
