More generally, a charge can be indicated (to the customer) as having been declined by AmEx when AmEx never saw the charge. This happens when the person processing the charge doesn't know what to do or how to request an approval. It can also happen on the first purchase after you've activated a new card, although this is less common than in the past. If you call AmEx, they can tell you instantly whether a charge was, in fact, declined.
