It's almost always some combination of less than ideal conditions or something mechanical and an incorrect or poor choice by the crew. Sometimes it's all one or the other other but usually a combination. It's super scary to dig so deep into these things but does reaffirm just how rare crashes are and how several things have to go wrong most times.

Crew Error
Ignorance (never knew or trained for that etc.)
Failure to follow correct action as published/taught
Distraction (never double checked)

that's life for us all even on-the-ground
