What makes you so sure that human factor is now the mail culprit? Non-failure of navigation or monitoring instruments just rules out - at this point in time - this and nothing else. Still in the game are also structural failures, maintenance issues, design flaws, ... Examples would be
Boeing 737 rudder issues,
Alaska Airlines Flight 261 or
Continental Express Flight 2574
