What can happen is that the booking is set up ie flight availability held and payment authorises, but the ticket fails to issue.

I think this is the most likely scenario as it would prevent check in and could also give the ineligible for voucher message.

Generally without a ticket the payment wouldnt change from a pending status and would drop back. Also without a ticket the flight availability held will drop out.
