I have the "Lucipass" PDF which seems to have everything required under the "providing proof of your result" section except "name and civic address of the laboratory or testing provider that administered or professionally observed the test" since this was a self-administered test. Your test was not professionally observed, so it doesn't qualify, regardless of whether an address is included on the results.
If the airline agent who checks the result isn't paying attention, you may make it through. But if they
are paying attention, they shouldn't accept it.
