For me, it was prorated by number of days from when it was posted. So 100% back in first 30 days, but on day 31, you get $95 minus (31/365)*95. Some others have said it is by month, but mine was in the middle of the 3rd month after the fee posted, and my equation almost exactly matched what I got back.
