I'm very surprised that a phone agent has the ability to upgrade a passenger to biz without triggering an immediate add/collect. The potential for agents upgrading anyone for whatever reason (friends, favors, to get someone off the phone, genuine concern) is high. Presumably, these sorts of actions go automatically for audit, where they're rejected (which is probably what happened here).
