I don't really know on the finer intricacies of most of your questions, I'm sure someone more knowledgeable will be able to wade in. But, I also had IB flights not credit automatically, 7 days post flight I asked for the manual review and they credited 2 or 3 days later. This was in the week and not at a weekend if that matters.
