1. In general there are no usage fees after you buy the VGC/MCGC.
2. No. Most POS systems if not all restrict the use of the store gift card to buy other gift cards especially VGC/MCGC.
3. If you are talking about the MB VGC/MCGC, Vanilla VGC/MCGC, USB VGC/MCGC the answer is no on the cash back. If you are talking about BB or some other prepaid debit card then perhaps yes you can.
