I found out during a stay in 2019. Was working remotely back then and happened to be super busy, so I stayed in the room and declined housekeeping for two straight days. The third day, front desk called and said while they don't need to clean the room, they must send someone to inspect it. Sure enough, ten minutes later a manager came and did a very quick walk-around. She was very apologetic and explained it's to make sure nothing illegal is going on.
Nowadays they have a door hanger you can put out saying "no service," and in my experience they don't do a room check, even if that is out there for many days. I have a lot of data points.
Of course, if you don't arrive in the hotel, you can't put the door hanger out.
At the Hyatt Place Waikiki, I kept the do not disturb on for one day during covid, and they put a really aggressive pre-printed note under my door that said they would be in my room to check it out in the next 24 hours - like it or not! It barely stopped short of dropping f-bombs.
But they never checked the room anyway.
