Not sure what the cost is right now, but in late Nov we used https://venistatmobilelabs.com/ in Manhattan. I had read about them in a business journal. They came to our hotel (Intercontinental) and did the test in a quiet area of the lobby. We paid to get results in 24 hrs and it was around $190 per person. This may be higher now given the demand is greater now than back in Nov (pre omicron). This worked well for us. Did not need to wait in a queue. We uploaded everything to Etihad and got the Verified to Fly confirmation. They did mix up my name on the certificate (reversed middle and first), but they fixed it with a call. Be sure to review all the info the test-taking is writing down is exactly as it is in your passport. Hope this helps.
