It was a pad F ticket on Alaska transferred to UA so I'm assuming there are significant bonuses for the fare bucket.
Interesting to note that the wrong dates are on the statement.
That looks to be fare based somehow. I can't come up with any math that would generate those numbers based on mileage flown.
