It is a long debated point on what to tip in the Maldives. While mostly it revolves around what to tip the butlers, there is also the question of what to tip in restaurants. While there is disagreement on whether or not to tip, I don't think I have heard of anyone adding 20%. 20% in tip is the high end of the US standard, I doubt there is anywhere else in the world but the US where a tip of 20% would even be considered.
I would not tip in the Maldives unless for something truly above and beyond. If you feel like tipping, of course do it on a scale that you feel appropriate, but just bear in mind that the tips are not part of their wage calculation the same way it is in the US, so 20% would require the delivered service to be rather outstanding and amazing...
